Record Description : When a BMV Field Representative visits a deputy registrar office, he/she completes a Site Visitation Report (SVR) as the reports and functions of the agency are reviewed. The completed report is forwarded via the laptop computer to an Assistant Chief or Chief for review. The Field Representative also completes a Daily Activity Report (FRDAR). The report is updated daily. The report includes dates, times, locations and general tasks performed each day. The FRDAR covers the same two weeks as the pay period and is submitted to the Assistant Chief or Chief for review. The Field Representative also completes an Observation Report for each Saturday unannounced visit to a license agency. The report is filed in the district office and a copy is given to the deputy registrar.
