Record Description : In accordance with section 4503.44 of the Ohio Revised Code, a person may apply for a disability placard if they meet at least one of the following conditions: 1. Cannot walk two hundred feet without stopping to rest; 2. Cannot walk without the use of or assistance from a brace, cane, crutch, another person, prosthetic device, wheelchair or other assistive device; 3. Is restricted by lung disease to such an extent that the persons forced (respiratory) expiratory volume for one second, when measured by spirometry, is less than one liter, or the arterial oxygen tension is less than 60 millimeters of mercury on room air at rest; 4. Uses portable oxygen; 5. Has a cardiac condition to the extent that the persons functional limitations are classified in severity as Class III or Class IV according to standards set by the American Heart Association; 6. Is Severely limited in the ability to walk due to an arthritic, neurological, or orthopedic condition; 7. Blind. Applicant may apply to the Bureau of Motor Vehicles, Registration Division (Mail In Unit) or any Deputy Registrar Agency for issuance of a disability placard which entitles the individual to park a motor vehicle in any special parking location reserved for disabled persons. The applicant must complete the application stating name of the disabled person, signature of disabled person or next of kin, street address, city, state, zip, county, social security number, date signed and telephone number. In addition the applicant must submit a prescription from his/her physician or chiropractor stating the disabled persons name, current date, that a disability placard is being requested, the duration of the disability, and the physicians or chiropractors signature. Disability Placards expire on the date stated on the prescription, and must be renewed accordingly. Processing fees are $3.50 per placard. (Applicant’s may apply for one additional placard by stating a reason as to why two placards are needed.) For renewal, an “Application for Renewal of Parking Placards for Persons with Disabilities” is mailed to the customer 45 days prior to the expiration date. The application must be signed by the applicant or next of kin. A prescription from the physician or chiropractor stating the above information is required for renewal.
