Record Description : The form is completed by the Special Plates Division and lists all chargeable and/or accountable items (license plates, validation stickers, etc.) that were transferred from one BMV division to another. The receipt contains the releasing and receiving divisions, towns, counties, item code, starting and ending numbers of what was transferred, reason for transfer, total, and signatures of the releasing/receiving divisions. DISTRIBUTION: Original - Deputy Registrar Services Division, Audit Section; Duplicate - Deputy Registrar Services Division; Triplicate - Special Plate Division (Releasing Division); Quadruplicate - Receiving Division; Quintuplicate - Destroyed, no value; Sextuplicate - Destroyed, no value.
