Record Description :  Under Chapters 4517 and 4738 of the Ohio Revised Code, a person may make application for various licenses issued by the Dealer and Salespersons Licensing Unit. The file contains the following information: 1. Dealer License Applicant Files: When a dealer, leasing dealer and/or salvage dealer wants to establish a place of business, he/she must make application to the BMV. 2. Replacement Plate/Validation Sticker/Registration Card Application: The application is completed by the motor vehicle dealership and forwarded to the Dealers and Salespersons Unit for issuance of a replacement license plate/validation sticker/registration card. The form is used when either the plate, sticker or card is lost, stolen or damaged. The application contains license plate number, validation number, name, address, reason for application (lost, stolen or damaged), police notification and applicants signature. The information is keyed into the computer system for easy reference. 3. Statement of Contract - Required by Section 4517.04 . Applicants for new motor vehicle dealer licenses must submit a copy of their agreement with the manufacturer to sell new motor vehicles. This record series contains copies of franchise agreements from all motor vehicle dealerships and is entered into the Dealers computer system. 4. Canceled Dealer and Salvage Licenses: This file contains applications for salvage, leasing or dealer licenses that were canceled voluntarily by the dealership or canceled by the BMV due to the applicant not following the requirements or non-compliance as stated under Chapters 4517 and 4738 of the Ohio Revised Code. No Motor Vehicle Dealers Board action is taken and their fees are refunded. These records are maintained for proof that the applicant previously applied for either a dealer, leasing or salvage licenses.
