Record Description : The log sheet is filled out daily by the employees in the Communication Section for recording all incoming and outgoing phone calls. The log is used for showing the employee''s work production (quantity of phone calls per day) and as a backup to prove that the problems and/or service repairs on the Deputy Registrars'' terminals were reported. The log contains the date, operator''s initials, Deputy Registrar''s agency number, from, to, time, problem and referred to.
