Record Title :  Audit of Continuing Education Requirements

Record Description :  The State Board of Emergency Medical, Fire, and Transportation Services (EMFTS) and the executive director of the Division of Emergency Medical Services (EMS), with the advice and counsel of the Firefighter and Fire Safety Inspector Training Committee of the EMFTS Board, may audit a certificate holder to ensure compliance with continuing education (CE) requirements. Individuals selected for audit are required to complete an audit form and provide documentation (including copies of CE certificates) and affirmation of all CE completed during the previous three-year certification cycle.

Retain audit records for seven years from the date the audit is successfully closed or the date the audit material is made available to investigation, then destroy. Beginning July 2021, records received as paper documents will be uploaded and retained as machine readable records in EMS Database at the Audits lab. Records for audits that are not successfully closed are made available to the Investigations section and become part of SAN 760-0977. Pursuant to ORC 4765.11 and 4765.55, the EMFTS Board and its executive director may establish certification and CE requirements for certificants. The standard certification cycle is 3 years; certificants may be audited. A CE course completed during an extension period cannot be used to meet CE requirements in the next certification cycle. Audits for an extended certification cycle or a failed audit may require access to records from both the current and previous certification cycles. Paper Shred

The Audit Excel spreadsheet is created and maintained by EMS staff as a working document to log audit information, track the progress of audits, and sort and compile information used to determine audit and record management decisions. The current Audit Excel spreadsheet supersedes previous Audit Excel spreadsheets which are maintained as records.
