Record Description : The Community Access Model waiver is a Medicaid home and community based waiver that has been developed in accordance with 1915(c) of the Social Security Act. It is, in part, a response to the planned closure of two state operated developmental centers, Applec Creek and Springview. The waiver will provide residents the opportunity to live in the community. The record series includes decision memos, concept papers, administrative rules, the waiver application and related materials.
