Record Description : The Withdrawl Reqest Form allows the owner of the account to withdraw funds to be used for college or as a refund. These forms indicate due dates and amount to be paid to the college, banking information of the account owner and hardship records. Forms would include: Prepaid Tuition Use Forms, Qualified Withdrawal Request Forms, Non-qualified Withdrawal Request Forms, Hardship Withdrawal Requests, Withdrawal Request Forms and all supporting documentation.
