Remove form from personnel file (Agency Series No. PER-8C, Authorization No. 580-1057) before destruction of the file and retain form for an additional 40 years, then destroy. If forms are not retained in the personnel file, then retain 40 years after termination of employment, then destroy. H.B. 383 (119th GA) allows PERS members to purchase service which was previously covered by a valid exemption under PERS. These previously exempt employees may now purchase exempt service if they later become members of PERS and have at least 18 contributing months of PERS service credit. The employer must research personnel records and locate the exemption forms. Employers unable to produce the forms are liable for the cost of the credit, therefore, we wish to retain the forms longer. Paper Shred
