Record Description :  The Ohio Cancer Incidence Surveillance System (OCISS) receives cancer reports from cancer reporters (for example, hospitals, physicians, ambulatory health care providers, laboratories) either in an electronic format, on paper, or via fax in accordance with Ohio law and rules that require the reporting of this information.

Five years from date of electronic receipt. At the end of each calendar year (for example, December 31, 2021), delete electronic cancer reports received more than 5 years prior (for example, on or before December 31, 2016). OCISS receives cancer reports from most cancer reporters (for example, hospitals, physicians, ambulatory health care providers, laboratories) in an electronic format. Within 5 years of receipt, electronic cancer reports will have been processed, audited, and any identified issues resolved. All electronic cancer reports will have been stored permanently in the main OCISS database. Electronic Erase
Three years after the date of diagnosis listed on paper or faxed cancer reports. At the end of each calendar year (for example, December 31, 2021), shred paper cancer reports and delete faxed cancer reports with a date of diagnosis more than 3 years prior (for example, on or before December 31, 2018). OCISS receives cancer reports from some cancer reporters (for example, hospitals, physicians, ambulatory health care providers, laboratories) on paper or via fax. Within 3 years of date of diagnosis, paper and faxed cancer reports will have been entered into an electronic system and any identified issues resolved. All electronic cancer reports are stored permanently in the main OCISS database. Paper Shred
