OCISS receives cancer reports from most cancer reporters (for example, hospitals, physicians, ambulatory health care providers, laboratories) in an electronic format. Within 5 years of receipt, electronic cancer reports will have been processed, audited, and any identified issues resolved. All electronic cancer reports will have been stored permanently in the main OCISS database. |

Three years after the date of diagnosis listed on paper or faxed cancer reports. At the end of each calendar year (for example, December 31, 2021), shred paper cancer reports and delete faxed cancer reports with a date of diagnosis more than 3 years prior (for example, on or before December 31, 2018). |
