OCISS receives cancer reports from most cancer reporters (for example, hospitals, physicians, ambulatory health care providers, laboratories) in an electronic format. Within 5 years of receipt, electronic cancer reports will have been processed, audited, and any identified issues resolved. All electronic cancer reports will have been stored permanently in the main OCISS database. |

At the end of each calendar year (for example, December 31, 2021), shred paper cancer reports and delete faxed cancer reports with a date of diagnosis more than 3 years prior (for example, on or before December 31, 2018). |
