Record Description :  The trial judge of any court of record shall suspend or revoke the driver's license of any person who is convicted, or pleads guilty to, any violation as listed in section 4507.16 of the Ohio Revised Code. The Conviction Cases are records of convictions or bond forveitures (Form UTT - Uniform Traffic Ticket). The tickets contain the violator's name and address, license number, date, location, type of violation, court's findings, sentence imposed and conviction date. The cases also contain insurance certificates or bonds evidencing financial responsibility and all correspondence relating thereto. These documents combine to establish cases which are scanned onto the optical imaging system in the Safety Responsibility Division of the Bureau. NOTE: Effective March 16, 1983, any person of a second offense D.U.I. (driving under the influence of alcohol and/or controlled substances) or any first time major conviction as specified under section 4507.16 of the Ohio Revised Code, is required to pay a reinstatement fee of $125.00 and maintain proof of financial responsibility insurance for 3 years from the date of suspension.

Effective September 1990 all cases generated will be recorded onto the optical imaging system. All documents are scanned onto the optical imaging system, reviewed then verified. The hard copies are then destroyed. Paper Wastepaper
Any cases generated prior to January 1989 will remain on microfilm/microfiche if not updated onto the optical imaging system. The microfiche will not be duplicated for storage at the State Records Center. The cases are retained for three (3) years from date of suspension, then destroyed providing all reinstatement requirements have been satisfied. Microform Burn
The disks will be duplicated and stored at the State Records Center. The case will be retained for three (3) years from date of the suspension, then erased from the disk providing all reinstatement requirements have been satisfied. Electronic Erase
